They assist in patient intake screening, patient counseling and education as well as discharge planning. Social workers are integral part of a multidisciplinary team that includes doctors, nurses and other healthcare providers and help them to understand the social and emotional aspects of a patient’s diagnosis and provide critical services that are focused on promoting healthy lifestyles and addressing any barriers to accessing health care.
